Consumer Assistance Contacts for U.S. States and Territories
You can get additional help if you don’t agree with a decision made by your health plan. If you decide to appeal, you can also get help with the appeals process.
To get assistance:
1. Contact your employer or the toll free number on the back of your medical plan ID card to obtain the following information:
- Find out if your medical plan is insured or self funded
- Find out the contract state for your medical plan if it's insured
2. If your medical plan is self funded, use the list below and select the state that you live in. If your medical plan is insured, then select your contract state from the list below.
In addition, there is help available from the Federal Government. The Health Care Reform law includes rules about appeals. Many plans have to follow these rules. If your plan is covered by the law*, you can contact the Employee Benefits Security Administration for help at 1-866-444-EBSA (3272). You can also send an e-mail/go to www.askebsa.dol.gov
